RiverView Health will offer sports physicals to all area high school athletes at a reduced cost in their Crookston, Fertile, Red Lake Falls and East Grand Forks clinics. Cost for the physicals will be $25.
Exams will be conducted at the Crookston Family Clinic on July 20, 27th and August 5th from 4:30-6:30 pm. No appointments will be taken. Students will be taken on a first come, first serve basis.
Exams at the Fertile Clinic will be offered during regular clinic hours July 26th — July 30th and August 9th — 13th. Call 218-945-6695 to make an appointment.
Red Lake Falls Clinic will offer the sports physicals July 28th during regular clinic hours, and July 29th until noon. Call 218-253-4606 to make an appointment.
East Grand Forks Clinic will offer sports physicals during regular clinic hours. Call 218-773-1390 for an appointment.
Students should bring their completed 2010-2011 Minnesota State High School League Health History to the exam. Extra health history forms are available at each of the RiverView Clinics.
